Novant Health Community Health Services

Childhood Immunizations: Our flagship Hemby mobile unit, provides immunizations (vaccines) to children ages 0-18 who are uninsured, underinsured, or Medicaid eligible in the greater Charlotte area. 

Adult Health Screenings: Our RemarkableYou program (age 18 and over) offers blood pressure, cholesterol, and pre-diabetes checks at no-charge by helping community members know their numbers to prevent serious health events and increase access to healthcare services. These screenings aim to serve those who are uninsured, underinsured, have no primary care physician or medical home, and those who have undiagnosed prediabetes or diabetes.

Community Health Education: our program provides learning experiences on health and special interest topics. The information presented is tailored to provide information based on your community’s need. We also partner with our Remarkable Novant Health teams. They have a passion to be the community providing education and resources aimed to improve health and awareness of the families we serve.

Please email a copy of your child’s immunization records to communitycruiser@novanthealth.org, or text the records to 704-351-1703 before your appointment date.  Sending this information in advance can help to reduce extended wait times. 

Remarkable You Screenings: Free blood pressure, BMI, waist circumference, cholesterol, glucose, A1C, triglycerides. To request services, please complete the request form and email it to gwsmcecommunityoutreach@novanthealth.org. 

Adult vaccinations: To schedule this service, please complete the community request form and return to gwsmcecommunityoutreach@novanthealth.org 

Adolescent/Childhood vaccinations: Eligible participants are children, newborn to age 18. The cruiser will be used to meet the needs of children who are uninsured or underinsured. Those who have full insurance coverage should continue to see their primary care physician. 

Health Education: Certified Health Educator available for group presentations, classes, and informational sessions.
