NHRMC Physician Group – Primary Care continues to deliver our patients evidence-based, patient-centered processes that focus on highly coordinated care and long-term, participative relationships.

Being a Patient-Centered Medical Home provides efficient, coordinated and long‐term relationships between providers and patients which improve outcomes and experiences and reduce healthcare costs.

We are continuing to use the NCQA Patient-Centered Medical Home as our model of primary care. This combines teamwork and information technology to foster ongoing partnerships between patients and their healthcare providers, instead of approaching care as the sum of episodic office visits. Each patient’s care is overseen by clinician-led care teams that coordinate treatment across the healthcare system. NCQA research shows that medical homes can lead to higher quality, lower costs and can improve patient and provider reported experiences of care.

New Hanover Medical Group originally was recognized as a Level Three medical home in 2016, the highest level recognized by the NCQA. To earn recognition, New Hanover Medical Group demonstrated the ability to meet the program’s key elements, embodying characteristics of the medical home. NCQA standards aligned with the joint principles of the Patient-Centered Medical Home established by the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association. We have now expanded PCMH standards across all Primary Care locations and are currently seeking NCQA recognition at three additional sites.

NHRMC Physician Group is affiliated with New Hanover Regional Medical Center and features comprehensive primary care for all ages at numerous locations throughout New Hanover, Pender, Brunswick and Onslow counties.

All practices offer well care, chronic disease management, sick care and injury care for the whole family; same-day routine/urgent care appointments for established patients. Multiple locations offer onsite diagnostic lab and radiology services. NHRMC Physician Group patients have access to NHRMC MyChart, a shared, secure electronic health record that connects patients to their health information anytime, anywhere.
