We want to meet your cardiac care needs. Please tell us the reasons for your visit when you schedule an appointment so we can give the right amount of time with your doctor.

Your Appointment

Please bring the following to each appointment:

  • Photo ID
  • Health insurance card
  • Medications you take in their original containers, including over-the-counter
  • Copayment, if you have one

Try to arrive a little bit early to fill out any paperwork. Please call us if you run late unexpectedly, so we can make sure you are seen soon after you arrive. If you must cancel, let us know as soon as possible so we can offer the time to someone else.

If you are scheduled for a diagnostic test, please do not wear lotions or oils.

Unexpected Needs

Appointments are necessary to see a healthcare provider. We understand that you may have an unforeseen need and want to see one of our providers the same day. Call us when that happens. We want to meet your heart care needs.


If you have chest pain, call 911 and go immediately to a hospital. Do not delay getting help.

Insurance and Billing

We verify your health insurance at each visit. Please bring your insurance card.

  • Medicaid
  • United Healthcare
  • Tricare Prime


Copayments are payable at each visit. We accept MasterCard, Visa, American Express, and personal checks.

Other Charges Due at Appointment

If your insurance plan doesn’t cover a service you receive, you are responsible for the charges.

Charges for services not covered by your insurance plan or the estimated amount you owe, besides the insurance portion, is payable at your visit.

We will file an insurance claim after your visit. When the insurance company makes its payment, we will reconcile the amount and either bill for any remaining amount or refund the difference to you.

If you cannot pay your portion of a service on the day of your visit, please speak with our business office before your appointment to make arrangements.
