Provider of Excellence
A Provider of Excellence is one who engages others and is a leader in clinical and service excellence. Demonstrating passion, commitment, and measurable outcomes through provider and patient satisfaction, as well as clinical performance, this individual defines the highest standards of medical care. The Provider of Excellence will be chosen twice per calendar year.
Any provider on the Medical Staff, including employed providers, as well as any provider who participates in the ACO is eligible.
Any employee of the organization or provider of the Medical Staff may nominate a Provider of Excellence. The Provider Excellence Panel meets twice per year to review, discuss and choose a winner based on the information contained in the online nomination form.
- Any provider on the NHRMC Medical Staff
- Any provider who participates in the ACO
- Any provider employed by NHRMC
- The term "provider" applies to any of the following types of medical staff members:
- Physician Assistants
- Nurse Practitioners
- Certified Nurse Midwives
The Provider of Excellence Panel selects the provider based on information contained in the nomination form. Any employee of the organization or any member of the Medical Staff may nominate a provider. The Excellence Panel meets twice per year to review, discuss and choose the winner. There must be a quorum (4) present to select a winner. The Provider of Excellence Panel is composed of:
- Chief Executive Officer
- Director of the Medical Staff Office
- Physician leaders
- Prime dedicated parking space for one full calendar year
- Purple name badge
- Desk plaque
- White Service Excellence polo shirt
- Recognition at LDI
- Other internal and external recognition

The provider you are nominating must demonstrate a commitment to Service Excellence and consistently exceed expectations related to the standards of performance (Ownership, Teamwork, Communication and Compassion). Please give specific examples of the way in which your nominee exemplifies these standards.
