Verbal Orders for Patient Testing
Inpatient verbal orders should not be called to the Laboratory. Inpatient verbal orders from a physician should be called to the nursing unit and nursing unit will enter orders in the computer. Nursing unit personnel will be responsible for making sure orders are signed on chart.
Outpatient verbal orders should only be called after office hours and must be called to a Medical Technologist in the Laboratory. During office hours, faxed orders should be sent for order to be placed. The technologist will complete a verbal order form. Personnel receiving verbal or phone orders must read back the entire order to verify accuracy of transcription. The technologist will instruct the physician to fax a written order to the Laboratory within 24 hours. Fax numbers to be used are 910.815.5565 (New Hanover Regional Medical Center), or 910.452.8273 (Cape Fear Memorial Hospital).
Faxed or written orders sent to Laboratory must include the patient's full name, complete billing information, social security number, physician's full name and ICD-9 code or diagnosis.
Technologist must forward verbal order, within 24 hours, to Laboratory Support Associate or Technical Assistant in the outpatient clinic area at New Hanover Regional Medical Center File it in the outpatient orders box at the front desk at Cape Fear Laboratory.
Outpatient clinic support associate, technical assistant or technologist will attach faxed orders to verbal order and forward to medical records. If the faxed/written order is not received within 24 hours, the support associate will make four attempts at contacting the physician and getting the written order within the next 30 days. Forms will be maintained in a file in work area. Documentation of attempts to get written order will be recorded on the verbal order form. If written order is not received within 30 days, the verbal order form will be forwarded to medical records.
