Your Patients' Care at NHRMC

We appreciate your allowing us to help care for your patients. We want to make the process of transferring, scheduling and consulting as easy as possible for you, and for them. 

Our healthcare teams also work to make your patients' care as seamless as possible and to coordinate services for hospitalized patients and those receiving services at NHRMC.

Incidental Lung Nodule Program at NHRMC

NHRMC continues to lead our community to outstanding health with the implementation of a program to manage incidental lung nodule findings. Incidental findings are discovered unintentionally during imaging exams. Radiologists will follow the Fleischner Society Guidelines for management of solid nodules, recommendations will be included in the report.

A Program Navigator will work/communicate directly with referring providers and patients to ensure incidental findings are acknowledged and receive proper follow-up care.

The incidental lung nodule program will complement the existing Lung Cancer Screening and multidisciplinary Lung program designed for newly diagnosed lung cancer patients.

NHRMC Transitional Care Clinic 

The NHRMC Transitional Care Clinic cares for patients who cannot connect to a medical home within seven days of hospital discharge.

Our Transitional Care providers work with you to help bridge any gaps in care and help provide necessary follow-up care until they can establish a relationship with a primary care office. We encourage you to look over some FAQs about the Transitional Care Clinic.
