You can manage your information at home by logging into Employee Self Service through Remote Access.

NHRMC expects employees covered by one of the organization's medical plans to have a brief health assessment that includes height, weight, blood pressure, total cholesterol, HDL and Hemoglobin A1C.  
Our Employee Health & Clinic will provide you with a free health risk assessment or you can see your personal primary physician to have the necessary lab testing completed and present the results to the Employee Health & Clinic. Annual Health risk assessments (HRA) are typically completed between 11/1 and 1/31. Outside lab test results collected between 10/1 through 1/31 will be accepted for the annual HRA.
The results are confidential and will not be used to determine the cost of health care coverage; however, we will use the aggregate information to further develop a comprehensive wellness program that will help our employees improve their health and reduce medical plan costs.

To qualify for the lower premium to avoid the $20 surcharge per pay period, complete the HRA.  Non-compliant Health Risk Assessment rates are within the guidelines established by the DOL, EEOC, ADA, and PPACA. Only the employee is responsible for completing the health risk assessment - not covered dependents. Join NHRMC in taking proactive steps to help employees effectively prevent or manage disease.
