We respect each employee's right to express his or her wishes about the care they wish to receive. That's why we're offering you another important benefit - providing help with completing Advance Directives free of charge.
Your Voice Your Choice
Advance directives - Healthcare Power of Attorney and Living Will - help you plan ahead and outline what kinds of medical treatments you would like in the event you become unable to speak for yourself. Having advance directives in place can give you peace of mind, knowing your loved ones will not be burdened by having to make these decisions if something were to happen to you.
Advance Directive Services Available to Employees and Dependents
Because we want you to have all the information you need to make and record these important decisions, we have been working with BeginTheConversation.org, a program offered through Lower Cape Fear Hospice, to help bring you comprehensive information on developing your plan. To help you complete your Advance Directives NHRMC is offering:
- Advance Directive forms (download and print)
- Required two witnesses and notary public to be present when you are ready to sign
- Filing of a copy of your Advance Directives to your medical record
If you have already completed your Advance Directives, you can drop them off at the Spiritual Care Department, and they will make sure they become part of your medical record.
