no appointment needed except for two and three hour glucose tolerance tests

Please complete the health questionnaire prior to arrival and bring it with you to your appointment. For your safety we must obtain an accurate list of your medicines and their doses as well an accurate medical and surgical history. This information is entered into your electronic medical record and is viewed by your surgeon and anesthesiologist in order to prepare you for surgery and provide continuity of your care following surgery.
