Coastal Family Medicine Center Recognized as a Medical Home
Coastal Family Medicine Center has been recognized by the National Committee on Quality Assurance (NCQA) for its success in creating a centralized "medical home" for patient care. This recognition, Patient Centered Medical Home (PCMH), is an approach to providing comprehensive primary care for children, youth and adults. Practices that are patient-centered medical homes act as care coordinators communicating between specialists, labs and other medical entities to ensure that their patients receive the highest level of care.
