Please utilize the work request and purchase requisition forms listed above. These online forms will replace the current paper forms that you may be using.
Only managers and night supervisors are allowed to submit requests. This is to ensure that the manager is aware of and approves all requests for their department since they may impact budgets.
These online forms MUST be used for all non-emergency requests. Conversations in the hallways do not constitute a request. Using email-based forms allows us to track all requests electronically so that they are not forgotten. For true emergencies, (ie patient/staff safety issues or hospital-wide outages) page or phone the appropriate department immediately.
Requests will be tracked by the Accountability team to ensure that they are being addressed in a timely manner. If you do not receive a response to your online request within 48 hours, resubmit or call the specific department for a status update. Every effort will be made to acknowledge receipt of your request and perform an initial assessment within two business days. If you do not get an acknowledgement of your request, assume that it did not go through and resubmit.
