The Opportunity: It’s arguably the
highlight of every patient’s hospital stay:
Going home. Some patients, however,
get out the door faster than others.
Delay in discharge can profoundly impact
hospital efficiency, not to mention
patient satisfaction. But with hundreds
of patients exiting our hospital a day,
how can we ensure each one gets – from
order to discharge – on their way as
quickly and safely as possible?
The Plan: Observing discharge from
start to finish, staff realized there was
no formal process for nurses to prioritize
patient discharges. By helping nursing
staff formalize a patient discharge
process – including asking patients their
discharge time preference and
establishing a standardized process for
discharge flow – long discharge wait
times could be avoided, getting patients
home faster.
The Outcome: Discharge wait times are
on a downward slope, with patients and
their families becoming increasingly
involved in the discharge dialogue. You
could say these improvements bring real
curb side appeal to our patient care.
