Remember to Record Time of Day in All Patient Record Entries

Please remember that it is important to include time of day in every entry in a patient record. Timing is critical in medical records. In most instances everyone records the date, but they are not always including the time. Time should be recorded on progress notes, on all orders, on every entry into a patient record, anytime a phone call is made, whenever verbal orders are taken over the phone, when signing off on an order, and anytime a critical test result has been relayed.
